Inductee Nomination Form – New Britain Sports Hall of Fame

This nomination form must be completed in its entirety for the nominee to be considered for induction into the New Britain Sports Hall of Fame. Any nomination form that is not fully completed shall not be considered for nominee induction. This nomination form must be received on or before October 1 of each calendar year so that the nominee may be timely considered for induction in the next calendar year. This nomination shall be considered on a yearly basis at the discretion of the New Britain Sports Hall of Fame Committee.

It is strongly recommended that information supporting the nominee shall be included with the submitted nomination so that the New Britain Hall of Fame Committee can fully consider the nominee. Nominee recognition, awards, letters of recommendation, newspaper articles and other documentation may be attached to this nomination form.
